Medical Social Worker – Case Management

Position Overview

The facility's care management department is seeking an experienced Medical Social Worker (MSW/LCSW) to join a collaborative team of clinicians focused on discharge planning and care coordination. You'll work with adult and geriatric patients across the hospital, managing complex care transitions and ensuring safe, timely discharge to appropriate levels of care.

Schedule & Contract

• Shift: Day shift, 5 days per week (Sunday–Thursday; subject to change based on staffing needs)
• Hours: 08:00–16:30
• Contract Duration: 13 weeks
• Start Date: ASAP
• Orientation: 40 hours

Clinical Scope

• Conduct initial assessments within 24 hours of admission
• Develop and coordinate individualized discharge plans
• Manage caseload of 20–23 patients
• Collaborate with medical teams, insurance case managers, and utilization management partners to place patients at the appropriate level of post-acute care
• Provide crisis intervention, biopsychosocial assessment, and care coordination across diverse diagnoses
• Document care delivery and patient/caregiver education according to plan of care

Requirements

• Minimum 1 year acute care hospital experience (required; redirect if not met)
• Current MSW or LCSW license
• BLS certification
• Travel experience required
• Compact license preferred at submission
• 2 professional references covering 1 year within the last 3 years

Important Notes

• You may be expected to float to nearby sister facilities; refusal may result in assignment termination
• Must commit to working 3 of 5 designated holidays (Thanksgiving, Christmas Eve/Day, New Year's Eve/Day)
• All RTO and scheduling requests must be submitted upfront during the application process
