Medical Social Worker – Acute Care
Position Overview
The facility is seeking a Medical Social Worker to join its acute care team on a day shift schedule. You'll work with a caseload of up to 25 patients daily, providing comprehensive social work and discharge planning services to adult and geriatric populations.
Schedule & Contract Details
- Shift: Day, 5×8 hours (8:00 AM – 4:30 PM)
- Contract Duration: 13 weeks
- Start Date: ASAP
- Weekend Requirement: Every other weekend (Saturday and Sunday), with a weekday off in exchange
- No floating required
- Orientation: 40 hours
Key Responsibilities
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team
- Conduct comprehensive discharge planning and needs assessments
- Order and coordinate durable medical equipment (DME)
- Organize and facilitate family meetings and care conferences
- Provide care coordination and support to patients and caregivers
- Navigate advance directives, POLST, and protective services processes
Required Qualifications
- MSW or LCSW license
- Minimum 2 years of acute medical social work experience, including discharge planning
- Experience working with diverse populations: patients with substance use disorders, experiencing homelessness, and those with mental health conditions
- Valid compact state license (required at submission)
- Travel experience required
Additional Requirements
- Maximum 7 days RTO (request upfront during application)
- Must commit to working 3 of 5 designated holidays
- Two professional references required (covering 1 year within the last 3 years)
- Background check required
