The facility is seeking a Case Manager to join a supportive, collaborative team in a healthcare setting recognized for clinical excellence and patient-centered care. This is a 13-week contract assignment beginning October 26, 2026.
About the Role
You'll work alongside RN and physician colleagues, managing patient cases with a focus on coordination, communication, and trusted relationships. This role requires strong organizational skills, the ability to prioritize multiple tasks, and excellent written and verbal communication. You'll need to build rapport with patients, families, and team members while maintaining detailed, accurate documentation.
