Case Management Registered Nurse
Location: North Charleston, SC
Shift: Monday–Friday, 8:00 AM–4:30 PM
Schedule: Every other weekend and holiday rotation
Contract Duration: 13 weeks
Required License: RN
About the Role
The facility is seeking a Case Management RN to join its acute care team. You will manage patient cases across hospital units, coordinating care transitions, insurance authorizations, and discharge planning. This is a clinically focused position where your advocacy directly impacts patient outcomes and hospital efficiency.
What You'll Do
- Conduct comprehensive case management assessments and develop individualized care plans
- Coordinate with clinical teams, social services, and external providers
- Navigate insurance authorizations and utilization review
- Facilitate safe, timely discharge planning
- Work collaboratively with 2 Case Management Assistants
What We're Looking For
- 1+ years of acute hospital case management experience (required)
- Current RN license
- Travel nursing experience preferred
- CCM or ACM certification preferred (not required)
The Work Environment
You'll work Monday–Friday with every other weekend and holiday rotation in accordance with facility policy. The department supports a collaborative approach to patient care with emphasis on teamwork and manageable workload.
