RN Care Coordinator – Emergency Department

Position Overview

The facility seeks an RN Care Coordinator for its Emergency Department. You'll manage 15–20 patient consultations daily, coordinating care, conducting initial assessments, and planning discharges for patients across all payer sources, including uninsured and underserved populations.

Shift & Schedule

• 8-hour shifts, 12:00 PM – 8:30 PM
• Every other weekend (minimum one full weekend per month)
• 40 guaranteed hours per week
• 13-week contract

Key Responsibilities

• Perform initial assessments and care coordination evaluations
• Complete utilization review and discharge planning
• Apply Interqual criteria to clinical and financial decision-making
• Collaborate with hospitalists, intensivists, respiratory therapy, and interdisciplinary teams
• Work with all payers and navigate complex insurance, financial assistance, and social determinant needs

Required Qualifications

• RN license (BSN required)
• Current BLS (AHA)
• Recent ED Care Coordination experience (within past 12 months preferred)
• Demonstrated familiarity with Interqual criteria
• Strong communication, collaboration, and prioritization skills

What You'll Need

• Ability to adapt and reprioritize throughout dynamic shifts
• Comfort working across all hospital areas (no outpatient or home care coverage)
• Business casual attire

Orientation & Support

• 5 days dedicated care coordination and new-employee orientation
• Unit-specific training based on experience
• On-site support: hospitalists and intensivists available 24/7

Cancellation Policy

Up to 4 shifts may be cancelled per 13-week assignment.
