Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Contract Duration: 13 weeks

Position Overview
The facility is seeking a Licensed Master of Social Work (LMSW) to join the discharge planning team. You will work collaboratively with an interdisciplinary care team to develop comprehensive discharge and aftercare plans that support safe transitions for patients leaving the hospital.
Key Responsibilities
- Conduct weekly assessments and care plan meetings with all patients on your caseload (adjusting frequency as clinically appropriate)
- Participate in team rounds and interdisciplinary care planning discussions
- Screen patients, complete assessments and documentation within established timeframes
- Develop and execute discharge plans, including referrals to home care and skilled nursing facilities
- Educate patients and families on discharge plans and aftercare resources
- Coordinate with community agencies and maintain key relationships to support continuity of care
- Ensure timely patient notification of discharge per departmental policy
- Manage referral and ordering applications for patient support services
Requirements
- Master's Degree in Social Work
- Valid Licensed Master of Social Work (LMSW) license issued by NYSED (primary source verification required)
- Minimum 1 year of LMSW experience
