Position Overview

The facility is seeking a licensed Physical Therapist to join its rehabilitation team. You will screen, evaluate, and rehabilitate patients with physical functioning disorders and complex wound care needs, developing individualized plans of care aligned with regulatory and clinical practice standards.

Key Responsibilities

• Perform comprehensive patient evaluations using standardized assessments and evidence-based practices
• Administer rehabilitation procedures including manual techniques, ambulation support, therapeutic exercises, modalities, and wound care
• Develop and adjust treatment plans based on patient progress and clinical outcomes
• Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
• Document all clinical information accurately in the electronic health record system
• Participate in patient care conferences, utilization reviews, and rehabilitation meetings
• Supervise and mentor physical therapist assistants, rehabilitation aides, and students
• Maintain compliance with APTA standards and Oklahoma state practice requirements

Requirements

• Current Oklahoma state physical therapy license
• Minimum 1 year of PT experience (new graduates with Level II clinical experience considered)
• Long-term care or rehabilitation setting experience required
• Strong communication and documentation skills

• Schedule: Days, 8-hour shifts (35 hours/week)
• Location: Purcell, Oklahoma
• Contract Duration: 13 weeks
