Case Manager – Acute Care

Position Overview

The facility is seeking a Registered Nurse Case Manager to coordinate care for a diverse patient population, from pre-admission through post-discharge. You'll serve as a liaison between patients, families, physicians, and the multidisciplinary care team, ensuring efficient, appropriate, and timely delivery of services.

Key Responsibilities

• Lead coordination of patient care with interdisciplinary teams, monitoring appropriateness and timeliness of services
• Develop and implement individualized care plans aligned with clinical needs and covered benefits
• Consult with physicians on resource utilization, treatment options, length of stay, and discharge planning
• Manage complex cases, including chronic pain, substance abuse, and high-utilization emergency department patients
• Coordinate patient transfers to tertiary centers with continuity of care documentation
• Conduct focused reviews on frequent ED visitors to identify care optimization opportunities
• Ensure compliance with regulatory and accreditation standards through data collection and reporting
• Collaborate with acute care case managers on ongoing care coordination

Requirements

• Active RN licensure (state of practice)
• Bachelor's Degree in Nursing (BSN) preferred
• Case management certification preferred
• Minimum 2 years recent case management or acute care experience
• Current BLS certification

Position Details

• Schedule: Day shift, 7:00 AM – 3:00 PM
• Duration: 13 weeks
• Hours: 36 hours guaranteed per week
• Start Date: November 23, 2026
• Location: Lodi, CA
