Care Coordinator RN — Case Management | Syracuse, NY
This 13-week contract places you in an acute care case management role, Monday through Friday, 8:00 AM–4:30 PM with no weekend rotation. The position is full-time at 40 hours per week.
The Role
You'll manage a caseload of 16–25 patients per day, focusing on acute care case management, discharge planning, and utilization management. This role operates differently from traditional case management positions — travelers are expected to function at full capacity following a 3-day orientation, so prior acute care case management experience is essential to your success here.
Required Qualifications
- Active, unencumbered New York RN license
- BLS certification
- Minimum 12 months of acute care nursing experience
- Acute care case management experience in a hospital setting
- Electronic health record proficiency required
Preferred Qualifications
- PRI and Screen certification
Schedule & Environment
- Monday–Friday, 8:00 AM–4:30 PM
- No weekend rotation
- Business casual or scrubs permitted
