Care Coordinator (RN) — Case Management | Syracuse, NY

This 13-week contract places an experienced RN in an acute care Case Management role focused on discharge planning and Utilization Management. This position operates differently from traditional case management — travelers should come prepared to work independently with minimal ramp-up time.

Shift & Schedule

• Days, Monday–Friday, 8:00 AM–4:30 PM
• 40 hours per week | No weekend rotation

Clinical Scope

• Acute care case management and discharge planning
• Utilization management with a focus on appropriate level of care and timely patient transitions
• Collaboration with multidisciplinary teams to coordinate safe, effective discharges
• Caseload of approximately 16–25 patients per day

Onboarding

Travelers receive 3 days of training and are expected to function at full capacity shortly after. Prior acute care case management experience is essential to succeed in this role.

Requirements

• Active, unencumbered New York RN license
• BLS certification
• PRI and Screen certification preferred
• Minimum 12 months of acute care nursing experience with acute care case management/discharge planning in a hospital setting
• Experience with electronic health record charting required
