Occupational Therapist — Skilled Nursing Facility | Monterey, CA

Contract: 13 weeks | Shift: Day, 7:00 AM – 7:00 PM | Guaranteed Hours: 38/week | Start Date: November 10, 2026

This SNF-based Occupational Therapist role is ideal for a clinician with hands-on skilled nursing experience who is ready to step into a collaborative, patient-focused rehab setting in Monterey, California.

Requirements:

• Minimum one year of SNF experience
• Active California OT license
• Familiarity with rehab documentation and outcomes tracking software (Rehab Optima platform)

Clinical Scope:

• Provide occupational therapy evaluation and treatment to skilled nursing residents
• Manage a caseload across varying acuity levels, including post-acute and long-term care patients
• Document patient progress and functional outcomes in compliance with SNF care standards
• Collaborate with interdisciplinary rehab and nursing teams to support individualized care plans
