An acute care facility in Los Angeles is seeking an experienced Care Coordination Social Worker for a 13-week contract. Candidates must have prior experience working within California Dignity Health systems to be considered.

Position Details

• Shift: 8-hour days, 7:00 AM – 3:00 PM
• Schedule: Every other weekend (EOW)
• Contract Duration: 13 weeks (91 contract weeks total)
• Guaranteed Hours: 40 hours per week
• Start Date: November 16, 2026

Role Overview

This position supports an acute care patient population through comprehensive care coordination, including discharge planning, resource navigation, and interdisciplinary collaboration. Candidates should bring solid, hands-on experience in care coordination within a hospital setting.

Required Credentials

• MSW or LCSW (either accepted)
• BLS Certification
• Active California license required at time of submission

Cancellation Policy

The facility may cancel up to 4 shifts during a 13-week assignment or up to 3 shifts for shorter assignments.

Key Requirement: Prior California Dignity Health experience is mandatory — candidates without this background will not be considered.
