Care Coordination RN — Glendale, CA

This contract role is an opportunity for an experienced Care Coordination RN to join an acute care facility in Glendale, CA. The position focuses on coordinating patient care across the continuum, requiring strong clinical judgment and communication skills to facilitate effective transitions and resource utilization.

Assignment Details

• Specialty: Case Management / Care Coordination
• Setting: Acute Care
• Shift: 8-hour Days, 7:00 AM – 3:00 PM
• Schedule: Every other weekend (EOW)
• Float: First to float
• Contract Duration: 13 weeks
• Guaranteed Hours: 40 hours per week
• Start Date: December 7, 2026

Cancellation Policy

The facility may cancel up to 4 shifts per 13-week assignment period.

Requirements

• Active California RN License
• Solid, hands-on Care Coordination experience
• Prior experience at a CA Dignity Health / CommonSpirit facility is required — candidates without this background will not be considered
• BLS certification (AHA)
• ACLS certification

This is a long-term contract well suited for a seasoned Care Coordination nurse who is comfortable floating and working within a large integrated health system environment.
