Patient Care Coordinator – RN | Night Shift | La Jolla, CA

Shift: Nights, 3×12-hour shifts (7:00 PM – 7:30 AM)

Contract Duration: 13 weeks

Start Date: November 16, 2026

Role Overview

This RN role focuses on care coordination and transition planning within a centralized transfer/care management function. You will screen patients in your assigned caseload to identify those who are medically vulnerable or whose needs may be better addressed through alternative care settings or specialized programs. You will collaborate with the interdisciplinary healthcare team and patient/family to develop individualized transition plans of care.

Clinical Background Required

• ICU or PCU experience preferred; strong MS or ED backgrounds considered
• Minimum 2 years of concurrent clinical experience in specialty

Required Credentials

• Current California RN License (Temp or Perm) in hand at time of submission
• AHA BLS certification

Additional Requirements

• Travel nursing experience required
• Electronic health record proficiency required within the past 6 months
• No local candidates within a 100-mile radius
• Maximum 7 days RTO; no new RTO requests after submission
• One supervisory-level reference required
• COVID-19 vaccination required (medical/religious exemptions considered)

Float & Scheduling Notes

• RNs may be required to float at or below their skillset, potentially to another campus, based on unit needs
• Non-billable learning modules are assigned periodically and must be completed within the designated week
