About the Role
The facility is seeking a Registered Nurse to provide skilled nursing care in a long-term care environment. You'll assess residents, manage chronic conditions, administer medications, coordinate wound care, and work closely with an interdisciplinary team including physicians, therapists, and dietitians.
Key Responsibilities
- Perform comprehensive resident assessments and monitor for changes in condition
- Administer medications and treatments per physician orders
- Manage chronic disease, wound care, and rehabilitation support
- Document care thoroughly, including MDS/PPS assessments
- Coordinate with the care team and educate residents and families on care plans
- Support infection control, fall prevention, and skin integrity protocols
- Participate in care planning meetings and regulatory compliance activities
Required Qualifications
- Active New York State RN License with Primary Source Verification from NYSED
- Associate Degree in Nursing (ASN/ADN) from an accredited program
- AHA BLS certification
- Minimum 1 year of skilled nursing or long-term care experience
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Experience with MDS assessments and geriatric care
