Case Management — Eureka, CA | Day Shift | 26-Week Contract
This Case Management role is based in Eureka, California, and is available for an immediate start. The position runs 5x8-hour day shifts (8:00 AM – 4:30 PM) on a 26-week contract.
About the Role
The facility's Case Management department manages a caseload of up to 25 patients spanning adolescent, adult, pediatric, and geriatric populations. You'll work alongside Social Services in an interdisciplinary model, coordinating care across the continuum.
Key Responsibilities
- Admission criteria review, utilization management, and discharge planning
- Appeals and denials, prior authorizations, and benefits eligibility
- Care coordination across ED, ICU, MS, SDU/PCU, and observation settings
- Experience with HMOs and IPAs required
- Familiarity with advance directives, POLST, and protective services processes
- CMS/DRG knowledge and Joint Commission/National Safety Goals compliance
Schedule & Expectations
- Day shift, Monday–Friday with every other weekend required
- No on-call required
- Floating between like units based on departmental need
- Must be available to work 3 of 5 major winter holidays
Requirements
- Minimum 2 years of acute care hospital Case Management experience, current within the last year
- Active RN or CM licensure; compact license must be in hand at submission
- Local candidates (within 50 miles) accepted
- Two professional references covering the last 3 years required
