Licensed Vocational Nurse (LVN) – Skilled Nursing Facility

Position Overview

The facility is seeking an LVN to provide individualized nursing care for skilled nursing facility residents. You'll work under the direction of the charge nurse and collaborate with physicians, nursing staff, residents, and families to deliver high-quality patient care.

Shift & Schedule

• 5 x 8-hour day shifts
• 13-week contract
• 36 guaranteed hours per week
• Start date: November 10, 2026

Required Qualifications

• Current California LVN license
• Current BLS level CPR certification
• Strong physical health and stamina (ability to walk, bend, lift, and perform manual patient care tasks)
• Excellent assessment and communication skills
• Dependable attendance and reliable work history
• Ability to manage challenging patient behaviors professionally
• Proficiency in English, both written and verbal

Key Responsibilities

• Conduct individualized patient assessments and report abnormal findings to the charge nurse and physician
• Develop and implement personalized care plans in collaboration with the healthcare team
• Administer medications and treatments accurately and timely per physician orders
• Perform nursing procedures (EKGs, blood draws, IV starts, wound care) following facility protocols
• Maintain comprehensive, timely documentation of patient care and responses
• Respond to emergencies and initiate appropriate resuscitation measures
• Maintain infection control and safety standards
• Participate in patient/family education and end-of-shift rounds
• Maintain confidentiality and provide respectful, non-discriminatory care

Dress Code

Caribbean Blue scrubs required.
