Case Management | Winchester, MA | Days | 13-Week Contract

This is a daytime Case Management position with a Monday–Friday schedule (8:00 AM – 4:30 PM), with occasional weekend availability as needed. No on-call or holiday requirements.

The Role

You'll work within a dedicated case management unit, focusing on care coordination and discharge planning across an inpatient setting. Social work coverage is handled separately, allowing you to focus on core case management functions.

Requirements

• 3–5 years of case management experience required
• BLS certification required
• Strong electronic health records proficiency required
• No floating required
• Candidates must be in good professional standing with prior employers; a minimum 6-month separation from previous affiliated employment is required for consideration

This is a stable, structured role for an experienced case manager seeking a consistent day schedule with no call burden and a long-term 13-week commitment.
