Medical Social Worker – Case Management

Position Overview

The facility's care management department is seeking a Medical Social Worker (MSW/LCSW) to join a multidisciplinary team providing discharge planning and case management for adult and geriatric patients in an acute care setting.

Shift & Schedule

• Day shift: 7:00 AM – 3:00 PM
• 5-day workweek (Sunday–Thursday, subject to staffing needs)
• 13-week contract
• Start date: ASAP

Key Responsibilities

• Manage a caseload of 20–23 patients
• Conduct initial assessments within 24 hours of admission
• Coordinate discharge planning and post-acute care placement
• Collaborate with medical teams, insurance case managers, and utilization review coordinators to transition patients to appropriate care levels
• Work with patients across a range of diagnoses and age groups
• Document care coordination in the EHR system per facility protocol

Required Qualifications

• MSW or LCSW license (California or compact license required at submission)
• BLS certification
• Minimum 1 year of acute care hospital experience
• Travel assignment experience required
• NOT accepting social workers specialized in behavioral health or ED settings

Notes

• Float availability to nearby sister facilities may be required
• Flexible scheduling; days off may not be consecutive
• Must commit to working 3 of 5 designated holidays
