Acute Care Case Manager – 26-Week Assignment
Position Overview
The facility is seeking an experienced Acute Care Case Manager to join its care coordination team. You'll work day shift (7:00 AM – 3:00 PM), five days per week, managing a caseload of approximately 25 patients across all age groups—neonates through geriatrics.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care across departments and facilitate family meetings
- Arrange appropriate post-acute placements (SNF, DME, ambulance transport)
- Manage utilization review and insurance authorizations (HMO/IPA knowledge required)
- Navigate complex psychosocial needs: homelessness, substance abuse treatment, mental health crises, foster/adoption situations, advance directives, and protective services
- Document thoroughly in computerized systems
- Collaborate with interdisciplinary teams including Physical Therapy and Social Services
Requirements
- Minimum 2 years acute care case management or discharge planning experience in a hospital setting (behavioral health experience does not substitute)
- Valid California nursing or case management license; compact license must be in hand at submission
- Seasoned travel experience required—first-time travelers not considered
- Strong organizational and communication skills
- Ability to work every other weekend to meet department needs
- Commitment to working 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Contract Details
- Duration: 26 weeks
- Start: ASAP (November 2, 2026)
- Orientation: 40 hours
- RTO Cap: 7 days maximum
- Scrub Code: Business attire or scrubs
