Acute Care Case Manager
Position Overview
The facility is seeking an experienced Acute Care Case Manager for a day shift position. You'll manage care coordination, discharge planning, and utilization review across a diverse patient population spanning neonates through geriatrics.
Shift & Schedule
- Day shift: 7:00 AM – 3:00 PM, 5 days per week
- Every other weekend required
- 13-week contract
- Start date: November 2, 2026
Key Responsibilities
- Conduct patient assessments and develop individualized care plans
- Lead family meetings and coordinate interdisciplinary care with physical therapy and social services
- Arrange DME, SNF placement, and medical transport independently
- Process prior authorizations, manage DRG and CMS compliance
- Conduct utilization review and discharge planning across surgical, emergency, and ICU services
- Navigate HMOs, IPAs, and coverage determinations
- Support patients with complex social needs: homelessness, substance abuse treatment, foster/adoption services, mental health crises, and advance directives
Requirements
- Minimum 2 years acute care case management/discharge planning experience in a hospital setting
- Seasoned travel experience required
- Current California state license (pending licenses accepted if active by start date)
- Strong organizational and communication skills
- Ability to work independently and collaboratively
What to Expect
- Patient ratio: 1:25 (days), 1:25 (weekends)
- Business attire or scrubs
- Must commit to 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Onboarding modules required (approximately 16 hours, completed pre-start)
- Possible float assignments to nearby facilities
