Shift & Schedule: Day 4x10-hour (07:00–17:00) or Mid 4x10-hour (13:00–23:00)

Contract Duration: 13 weeks

About the Role

The facility seeks a Clinical Pharmacist to deliver comprehensive pharmaceutical care across inpatient and outpatient settings. You'll work within a collaborative healthcare team, optimizing medication therapies, ensuring patient safety, and participating in interprofessional rounds.

Key Responsibilities

  • Review medication orders for appropriateness, safety, and efficacy in both acute and ambulatory settings
  • Consult with physicians and clinical staff to optimize pharmacotherapy
  • Conduct medication reconciliation, patient education, and transition-of-care services
  • Monitor patient medication and nutritional therapies; intervene when indicated
  • Prepare compounded medications meeting USP standards and complete quality assurance reviews
  • Educate and mentor pharmacy technicians, interns, and residents
  • Respond to emergency alerts (Code Blue, Code Sepsis, Code Stroke, etc.) as assigned
  • Document clinical interventions and maintain accurate federal/state records
  • Participate in formulary review, medication use evaluation, and policy development as applicable to your specialty area

Specialty Tracks Available

Depending on assignment, you may specialize in: Emergency Medicine, Infectious Disease, Anemia Management, Critical Care, Home Infusion, Oncology, Ambulatory Care, or Family Medicine. Each track carries specific experience expectations—see qualifications below.

Required Qualifications

  • Current California Registered Pharmacist license
  • Pharm. D. degree
  • Minimum 1 year clinical hospital pharmacist experience or completed PGY-1 residency
  • 3 years total experience preferred

Specialty-Specific Experience

  • Emergency Medicine: 1 year ED or critical care pharmacy experience; residency in Emergency Medicine preferred
  • Infectious Disease: 1 year hospital-based clinical experience
  • Anemia Management: 1 year hospital or ambulatory care-based experience
  • Home Infusion: 1 year home infusion or clinical experience; residency in home infusion preferred
  • Critical Care: 1 year critical care pharmacy experience; residency preferred
  • Oncology: 2 years acute inpatient hospital experience; residency in Oncology preferred
  • Ambulatory Care: 1 year ambulatory experience or residency; BCACP, BCPS, or CGP certification preferred
  • Family Medicine: Residency in General Practice with ambulatory focus; BCACP, BCPS, or CGP preferred

Additional Requirements

  • Float within the facility as needed
  • COVID-19 vaccine required (medical/religious exemptions considered)
  • No prior charting system experience required; training provided
