Licensed Vocational Nurse (LVN) – Skilled Nursing Facility

Location: Alturas, CA

Shift: Days, 5×8 hours

Contract Duration: 13 weeks

Start Date: October 20, 2026

About the Role

You'll report to the Director of Nursing and work collaboratively with physicians, nursing staff, residents, families, and other departments to deliver individualized, high-quality care in a skilled nursing facility setting. You'll be part of a professional nursing team and are required to wear Caribbean Blue scrubs.

Key Responsibilities

• Conduct patient assessments and develop individualized care plans in collaboration with the healthcare team
• Perform physician-ordered treatments and nursing procedures (EKGs, blood draws, IV starts, wound care, etc.)
• Administer medications per facility policy and maintain accurate documentation
• Respond promptly to patient needs and maintain safe, clean work environments
• Initiate emergency protocols and respond appropriately in urgent situations
• Communicate patient conditions clearly during shift changes and with appropriate personnel
• Educate patients and families, and assist with staff orientation as needed
• Maintain strict confidentiality and uphold infection control and safety standards

Required Qualifications

• Current California LVN license
• Current BLS CPR certification
• Strong physical stamina (ability to walk, bend, lift, perform manual patient care skills)
• Excellent communication skills in English (verbal and written)
• Dependable attendance and professional demeanor
• Strong assessment and problem-solving abilities
• Ability to work independently and as part of a team
• Commitment to ongoing professional development
