Clinical Social Worker – Acute Care

Position Overview

The facility seeks a Master Social Worker to provide comprehensive clinical social work services, counseling, discharge planning, and case management to hospital patients and their families. You'll work collaboratively with physicians, nursing, and the interdisciplinary healthcare team to assess psychosocial needs and develop integrated plans of care.

Key Responsibilities

  • Conduct psychosocial assessments and document emotional, social, financial, and environmental needs
  • Develop and implement discharge plans tailored to each patient's unique strengths and circumstances
  • Provide counseling and case management services, with particular skill in supporting patients and families coping with loss and grief
  • Monitor progress toward care goals and identify barriers to optimal length of stay
  • Facilitate connections with community-based resources to support continuity of care
  • Collaborate with the care team to interpret social and psychological factors impacting patient outcomes
  • Maintain comprehensive medical record documentation

Requirements

  • Master of Social Work degree from a CSWE-accredited program
  • ASW (Associate Clinical Social Worker) license or higher; LCSW (Licensed Clinical Social Worker) preferred
  • Acute care experience required
  • Strong psychosocial assessment and critical thinking skills
  • Demonstrated ability to work with diverse populations across the lifespan
  • Excellent written and oral communication skills
  • Proficiency with word processing and databases (preferred)

Schedule

  • Full-time, 5 days/week, 8-hour days (8:00 AM – 4:30 PM)
  • Weekdays and weekends as assigned
  • 13-week contract
  • ASAP start date
