Case Manager – Acute Care
Position Overview
Key Responsibilities
- Perform patient assessments and develop individualized care coordination and discharge plans
- Participate in family meetings and contribute to treatment plan development
- Coordinate with interdisciplinary teams including social services and transportation
- Arrange DME, skilled nursing facility placements, and ambulance transport independently
- Support patients with complex needs, including mental health concerns, substance use history, and homelessness
- Document thoroughly using computerized systems and maintain proficiency with Microsoft applications
- Assist with advance directives, POLST forms, and protective services processes
Required Qualifications
- Minimum 2 years acute care hospital Case Manager experience (hospital-based experience is required; behavioral health experience alone does not qualify)
- California state license (pending licenses accepted if licensed by start date)
- Knowledge of HMO and IPA systems
- Familiarity with InterQual admission criteria
- Strong organizational and communication skills
Assignment Details
- Shift: Day, 5×8 hours (7:00 AM–3:00 PM)
- Duration: 13 weeks
- Start Date: ASAP
- Weekend Requirements: Weekend rotation as needed by department
- Floating: No mandatory float between facilities
- Time Off: Maximum 7 days RTO; must request all scheduling needs upfront
