Hospital Case Manager — Acute Care | Mission Hills, CA

Day Shift | 5×8 Hours (08:00–16:30) | 26-Week Contract

This acute care facility is seeking an experienced Hospital Case Manager to support care coordination and discharge planning across a diverse patient population including adolescents, adults, and geriatric patients.

Shift & Schedule

• Days: 8-hour shifts, Monday–Friday
• Weekend rotation required to meet departmental needs
• Holiday commitment: must work 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Clinical Scope

• Conduct admission criteria assessments and ongoing utilization review using InterQual criteria
• Develop and participate in individualized treatment and plan-of-care meetings, including family conferences
• Coordinate discharge planning across complex social situations — including patients experiencing homelessness, substance use, mental health crises, advance directives, and protective services needs
• Arrange DME, SNF placement, and ambulance transport independently
• Collaborate with interdisciplinary team including social services and transportation
• Experience working with HMOs, IPAs, and CMS guidelines required

Requirements

• Minimum 2 years of acute care hospital Case Manager experience (Care Coordination & Discharge Planning)
• Acute hospital experience is required; behavioral health-only backgrounds do not qualify
• Active California RN or Case Management license required (pending license accepted if active by start date)
• Proficiency in computerized documentation and Microsoft Office applications
• First-time travelers accepted; clinical screening will apply
• Float between nearby sister facilities may be required

Time Off: Maximum 7 days RTO; all requests must be submitted upfront at time of offer
