About the Role
The facility's Case Management team is seeking an experienced case manager to join their acute care operations. You'll manage patient caseloads across multiple units, coordinating care transitions, insurance authorizations, and discharge planning for a diverse patient population.
What You'll Do
- Manage 30–35 patients per assignment
- Work collaboratively across multiple units as needed
- Participate in weekday and weekend scheduling (day shift: 8:00 AM – 4:30 PM)
- Contribute to timely, effective patient transitions and care coordination
What We're Looking For
- Proven acute care case management experience required
- Strong ability to adapt quickly and work independently
- Expectation of full productivity within 5 days of orientation
- Current certification (if applicable to your state/role)
