Position Overview
The facility is seeking a Case Manager for a 13-week contract assignment. You'll work day shift, 7:00 AM – 3:00 PM, in a dynamic healthcare environment.
About the Role
You'll collaborate closely with RN and MD teams to coordinate patient care, manage transitions, and support optimal patient outcomes. Your responsibilities will include:
- Demonstrating strong documentation skills using established charting systems
- Communicating clearly and professionally with team members, patients, and families
- Managing multiple priorities and shifting attention between tasks as needed
- Building trust and rapport with patients, families, and care team members
- Supporting a patient-centered care model in a supportive, team-focused environment
Unit Environment
This is a busy, multipurpose healthcare facility serving as a regional referral center with a diverse patient population and high clinical acuity. You'll work alongside experienced staff who prioritize collaboration and excellence.
