Clinical Pharmacist Disease Specialist – HIV & Hepatitis C
About the Role
The facility seeks a Clinical Pharmacist Disease Specialist to provide clinical leadership and pharmaceutical care for ambulatory HIV and Hepatitis C patients. You'll work as a mid-level provider in a collaborative, multidisciplinary care environment focused on optimizing medication therapy and improving patient outcomes.
Key Responsibilities
- See approximately 14 patients per day in the outpatient setting
- Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements
- Perform drug utilization reviews and monitor for drug interactions and adverse events
- Minimize polypharmacy and create patient medication action plans with self-management goals
- Serve as a clinical resource to outpatient and inpatient clinicians on HIV and Hepatitis C pharmacotherapy
- Collaborate with the Inpatient Infectious Diseases Clinical Pharmacist to transition patients from hospital to outpatient care
- Preceptor pharmacy students and residents; participate in didactic and experiential education
- Function as a key member of the multidisciplinary care team
Schedule & Duration
- Monday–Friday, 7:00 AM–3:00 PM (40 hours/week)
- 13-week contract
- Onsite
Required Qualifications
- Doctorate of Pharmacy from an accredited College of Pharmacy
- 2 years ASHP residency in HIV
- 5 years of direct HIV patient care experience
- Current Texas State Board of Pharmacy license or eligibility
- BLS certification
- Flu vaccine required (medical/religious exemptions accepted)
