Clinical Pharmacist – Primary Care
Position Overview
The facility is seeking a Clinical Pharmacist to provide pharmaceutical care leadership across two primary care clinics. You'll work as a mid-level provider within a collaborative, multidisciplinary team focused on optimizing medication management and patient outcomes.
Schedule & Location
- 4x10-hour shifts: 07:00–17:00 or 09:00–19:00
- Weekdays only
- On-site in Arlington, TX
- 13-week contract
Required Qualifications
- BCACP board certification
- Completed PGY2 Ambulatory Care residency
- Texas State Board of Pharmacy license
- 4+ years of direct patient care demonstrating medication management experience
- BLS certification
- Flu vaccine required (medical/religious exemptions accepted)
Key Responsibilities
- See approximately 16 patients per day
- Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements
- Conduct drug utilization reviews and monitor for drug interactions and adverse events
- Minimize polypharmacy and create patient medication action plans with self-management goals
- Serve as a preceptor for pharmacy students and residents
- Provide drug information, dosing, and monitoring services to practitioners
- Present clinical interventions and medication action plans to the care team
- Develop educational materials and in-services for medical, nursing, and pharmacy staff
- Support P&T policy development and pharmacy safety initiatives

Busy primary care setting with extended daytime hours, emphasizing team-based care in a medical home model.
