Medical Social Worker

Position Overview

The facility seeks a Medical Social Worker to join its care coordination team. You'll work collaboratively with RN case management and clinical staff to support patients across adult, geriatric, pediatric, and medical/surgical populations.

Key Responsibilities

• Conduct psychosocial assessments to identify patient and family needs
• Provide supportive counseling and crisis intervention
• Coordinate discharge planning and service arrangements with internal and external agencies
• Collaborate with the interdisciplinary care team to ensure continuity of care
• crises and connect patients with appropriate resources

Requirements

• Master's degree in Social Work (MSW)
• Current Massachusetts Social Work licensure (LCSW/LICSW)
• Minimum one year of hospital-based social work experience or equivalent in a fast-paced, high-acuity setting
• Demonstrated ability to work collaboratively across disciplines and agencies
• BLS certification
• Flu vaccine required during onboarding
• Cannot have worked for any Beth Israel facility (permanent, travel, or PRN) within the past 6 months

Preferred Qualifications

Experience in protective services, child welfare, long-term care, crisis intervention, hospice care, substance use disorders, or psychiatric illness support is a plus.

Schedule & Contract Details

• 8-hour day shifts: 7:00 AM – 3:00 PM
• Includes weekends
• No float requirement
• 13-week contract with 72 guaranteed hours biweekly
• Start date: November 2, 2026
