Position Overview
The facility is seeking a Case Manager for a 13-week assignment. This is a full-time role working days (7:00 AM – 3:00 PM) with self-scheduling flexibility.
About the Role
You'll work collaboratively with the nursing and medical teams to support patient care coordination and transitions. This position requires strong documentation and communication skills, with the ability to manage multiple priorities simultaneously while building trust with patients, families, and staff.
