Case Management | Acute Care

This position is open to experienced Care Coordination RNs ready to step into a fast-paced acute care environment in Los Angeles. The role focuses on care coordination and case management across an acute patient population, requiring strong clinical judgment and organizational skills to facilitate efficient, high-quality patient throughput and discharge planning.

Shift Details

• 8-hour days, 08:30–17:00
• 2 weekends per month
• Guaranteed 40 hours per week

Contract Details

• Duration: 13 weeks
• Start Date: October 19, 2026
• Orientation: October 19 and November 2
• Cancellation Policy: Up to 4 shifts may be cancelled per 13-week assignment

Requirements

• Active California RN License
• BLS Certification
• LA City Fire & Safety Certification
• Solid, demonstrated experience in Care Coordination/Case Management
• Prior experience with CA Dignity Health / CommonSpirit facilities is required — candidates without this background will not be considered

This is a long-term engagement well-suited for a seasoned care coordination professional who is already familiar with the Dignity/CommonSpirit system and can hit the ground running with minimal ramp-up time.
