Hospice Nurse
Position Overview
Join the hospice care team in Grand Junction, CO, where you'll provide compassionate, high-quality care to patients and families during one of life's most meaningful transitions. As a Primary Registered Nurse, you'll manage your own caseload while leading an interdisciplinary team focused on comfort, dignity, and personalized care goals.
Responsibilities
- Assess and manage symptoms with clinical excellence and critical thinking
- Educate patients and families about goals of care and empower caregivers
- Provide nursing care in diverse settings: private homes, nursing homes, group homes, and assisted living facilities
- Lead and collaborate with the interdisciplinary hospice team
- Act as the primary point of contact for your assigned patient population
Schedule & Requirements
- Hours: Monday–Friday, 7:00 AM–3:00 PM
- On-call/Weekend: Occasional back-up rotations
- Contract Duration: 12 weeks
- Start Date: September 14, 2026
- License: Current RN license required
