Physical Therapist – Long-Term Care

Position Overview

The facility is seeking a Physical Therapist to join its long-term care team. You will assess patients, develop individualized treatment plans, and work collaboratively with patients and their families to restore mobility, reduce pain, and prevent disability.

Key Responsibilities

• Perform comprehensive patient evaluations, including history, systems review, and targeted tests and measures
• Develop and implement individualized, patient-centered treatment plans aligned with physician orders
• Conduct ongoing reassessments and adjust treatment as needed based on patient response
• Manage discharge planning from initial evaluation through transition
• Supervise ancillary staff and students in accordance with state regulations and department policy
• Document patient care clearly and accurately, meeting regulatory and accreditation standards
• Educate patients, families, and caregivers on functional goals and home exercise programs
• Participate in quality improvement and professional development activities
• Maintain a safe environment for patients, visitors, and staff
• Function within your scope of practice as defined by Texas state law

Requirements

• Texas PT license or compact license (required)
• Minimum 2 years of physical therapy experience in a long-term care setting (required)
• 8-hour day shift, Monday–Friday
• 13-week contract
• Located in Ranger, TX
