Position Overview
The facility is seeking an experienced Case Manager to join its acute care team. You'll manage a caseload of up to 25 patients, coordinating care across multiple patient populations including adolescents, adults, pediatrics, and geriatrics. This role focuses on discharge planning, benefits eligibility, care coordination, and appeals management.
Schedule & Logistics
- Shift: Day, 5×8-hour (8:00 AM – 4:30 PM)
- Duration: 26-week contract
- Start: ASAP
- Weekend Coverage: Every other weekend required
- Call: None
- Float: Yes, to like units based on departmental need
- Parking: Free
Requirements
- Minimum 2 years of acute care case management experience (must be current within the last year)
- Strong knowledge of HMOs, IPAs, advance directives, and POLST processes
- Competency with benefits eligibility, discharge planning, DRG concepts, and CMS guidelines
- Familiarity with protective services and prior authorization processes
- Current compact license required at submission
- 2 professional references (covering 1 year within the last 3 years)
