Inpatient Medical Social Worker
Position Overview
The facility is seeking an experienced Medical Social Worker to join the acute care team. You'll manage a caseload of up to 25 adult and geriatric patients, performing both social work and discharge planning functions in a fast-paced inpatient setting.
Shift & Schedule
- Days: 5×8 hours (7:00 AM – 3:00 PM)
- Contract Duration: 13 weeks
- Start Date: ASAP
- Weekend Requirement: Every other Saturday and Sunday, with a weekday off in exchange
- Holiday Commitment: Must work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Key Responsibilities
- Develop and implement individualized plans of care
- Conduct comprehensive needs assessments and coordinate DME ordering
- Lead and participate in family meetings and interdisciplinary care conferences
- Support patients with substance use, homelessness, mental health crises, and protective services issues
- Facilitate advance directive and POLST documentation
- Provide discharge planning and care coordination
Required Qualifications
- License: MSW or LCSW (or both)
- Experience: Minimum 2 years acute medical social work
- Additional: Strong organizational and collaborative skills; knowledge of community resources for vulnerable populations
