RN Care Manager
Location: York, ME
Contract Duration: 13 weeks
Shift Options: 8:30 am – 4:30 pm OR four 10-hour shifts, 7:30 am – 5:00 pm
Schedule: No call; weekend and holiday rotation required
Hours: 40 per week
About the Role
You'll serve as the primary link between patients, clinical teams, and payors, coordinating utilization management activities across the hospital. This role requires strong advocacy and communication as you guide patients through their care journey and transitions to the next level of care.
Key Responsibilities
- Assess patient needs and identify resources required for safe transitions to post-acute settings
- Lead and collaborate with interdisciplinary teams during daily care rounds and care conferences
- Communicate effectively via written, verbal, and electronic systems
- Partner with community organizations to secure durable medical equipment, insurance authorizations, and transportation
- Participate in length-of-stay management for inpatient populations
- Apply knowledge of hospital reimbursement and quality measures relevant to care management
Required Qualifications
- Associate or Bachelor degree-prepared RN
- 3–5 years clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review
- Utilization review and discharge planning experience required
- Active BLS certification
Preferred Qualifications
- CCM (Certified Case Manager) or ACM (Ambulatory Care Manager) certification
- Certification by a nationally recognized case management or utilization review organization
