Medical Social Worker — Inpatient Case Management

Day Shift | 3x12-Hour (08:00–16:30) | 13-Week Contract | Santa Monica, CA

This inpatient medical social work role supports a diverse patient population spanning neonates, infants, adults, and geriatrics, with a daily census of up to 22–25 patients. The position involves complex care coordination, discharge planning, and crisis intervention across multiple care settings, including the Emergency Department.

Shift & Schedule

• Days, 3x12-hour shifts (08:00–16:30)
• Weekend rotation: 2 weekends per 4-week period
• No on-call required
• Holiday commitment: must work 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Required Credentials

• LCSW or MSW (active, valid license required)
• Minimum 2 years of experience
• Prior travel experience required
• Compact license must be in hand at time of submission

Clinical Scope

• Biopsychosocial assessments and initial evaluations
• 5150/Involuntary hold processes (required for ED-facing responsibilities)
• Care coordination with HMOs, IPAs, and community-based services
• Discharge planning, DME coordination, and referrals to rehab or long-term care
• Crisis intervention, including suicide/homicide/self-harm risk assessment
• Family therapy and interdisciplinary care conferences
• Knowledge of adoption/foster care, protective services, homelessness resources, advance directives, and POLST

Additional Details

• Orientation: 40 hours
• Floating to nearby sister facilities may be required; refusal may impact guaranteed hours
• All RTO requests must be submitted upfront; maximum 7 days
• Two professional references required, covering at least 1 year within the last 3 years
