Acute Care Case Manager

Position Overview

The facility is seeking an experienced Acute Care Case Manager to join the care coordination team. You'll manage a diverse patient population across multiple units, with a focus on discharge planning and ensuring seamless transitions of care.

Shift & Schedule

  • Day shift: 8:00 AM – 4:30 PM, Monday–Friday
  • Every other weekend (may cover up to 2 units on weekends)
  • Must commit to 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
  • 13-week contract; start ASAP

Key Responsibilities

  • Manage a caseload of up to 25 patients across the facility
  • Conduct biopsychosocial assessments and develop individualized treatment plans
  • Coordinate discharge planning and arrange post-acute services (home care, equipment, rehab referrals)
  • Facilitate family meetings and high-risk interventions
  • Collaborate with interdisciplinary teams (IV teams, PT, respiratory, pharmacy, social services, rapid response)
  • Educate patients and caregivers on discharge preparation and ongoing care
  • Handle complex cases involving mental health crises, substance abuse, homelessness, and protective services

Required Qualifications

  • Master of Social Work (MSW)
  • Minimum 2 years acute hospital case management experience
  • Knowledge of 5150/involuntary holds, POLST, benefits/eligibility, and care coordination
  • Proficiency with electronic health records (charting system experience required)
  • Valid California state license (pending license accepted)
  • Travel experience required

  • Patient ratio: 1:35
  • Scrubs or business casual attire
  • Orientation: 40 hours
  • May be required to float to nearby facilities
