Skilled Nursing Unit – RN, LPN, and CNA Positions
Position Overview
The facility's Skilled Nursing Unit is seeking experienced RNs, LPNs, and CNAs to join a collaborative post-acute care team. You'll work in semi-private rooms caring for patients with complex pulmonary conditions, dementia, and Alzheimer's disease who require ongoing rehabilitation and skilled nursing support.
Shift & Schedule
- 12-hour days (7:00 AM – 7:00 PM)
- Every other weekend
- First to float to acute rehab and medical/surgical units
- Weekend and holiday requirements apply
- 36 guaranteed hours per week
- No self-scheduling or block scheduling
Clinical Scope & Key Responsibilities
- Medication administration and wound care
- Tracheostomy and IV care
- Participation in interdisciplinary team conferences
- Collaboration with on-site respiratory therapy, physical therapy, occupational therapy, and speech-language pathology
Required Qualifications
- RNs & LPNs: BLS (AHA), ACLS (AHA), NIHSS (AHA or APEX)
- CNAs: BLS (AHA)
- Prior SNF or subacute experience required
- Valid license in California
Unit Details
- On-site respiratory therapy (24/7), pharmacy, and physician availability after hours
- Stable vents and tracheostomies (no titratable drips)
- Unit-specific orientation provided
- Both PAPR and N95 masks available
