Physical Therapist
Location: Ukiah, CA
Shift: 8:00 AM – 4:30 PM, Monday–Friday (no weekends)
Contract Duration: 13 weeks
About the Role
You'll join a healthcare facility offering comprehensive rehabilitation services, including physical therapy, intensive care, and advanced wound care. This position supports an interdisciplinary team delivering patient-centered care across acute care and rehabilitation settings.
Key Responsibilities
- Conduct patient evaluations and develop individualized treatment plans based on clinical assessment
- Provide evidence-based therapy interventions using appropriate protocols and techniques
- Establish discharge plans and communicate care coordination with the care team
- Maintain timely, accurate clinical documentation
- Supervise assigned physical therapy assistants
- Serve as a collaborative team member in interdisciplinary patient care
Requirements
- Doctor of Physical Therapy (DPT) degree required; if obtained after January 2016, must hold current state licensure
- Current Physical Therapy licensure in California
- Current Basic Life Support (BLS) certification
- Minimum 2 years of Physical Therapy experience preferred; 3–5 years preferred
- Pelvic floor therapy experience required; PRPC or WCS certification (or active pursuit) required
- Electronic health record (EHR) experience required
