Care Manager – 13-Week Contract

About the Role

The facility seeks an experienced Care Manager to support acute inpatient populations. You'll assess, plan, implement, monitor, and evaluate care options and services for 18–22 patients, managing length of stay and promoting efficient resource utilization through collaborative practice.

Key Responsibilities

• Perform comprehensive assessments of physical and mental impairment status
• Evaluate medical necessity and appropriateness of admission using evidence-based screening tools
• Navigate benefits, plan contracts, eligibility requirements, and utilization management protocols
• Apply current clinical practice standards and acute inpatient care methodologies
• Communicate effectively with patients, providers, and multidisciplinary teams to ensure quality, cost-effective outcomes

Requirements

• BSN required
• Minimum 3 years of healthcare experience with demonstrated knowledge of acute inpatient care and assessment methods
• Willingness to float as unit needs require

Preferred Qualifications

• 5+ years of experience in Utilization Management and Case Management
• CCM or related certification

Schedule & Duration

• 4 × 10-hour shifts (40 hours/week)
• Days; no set day off
• Every 6th weekend as needed
• 13-week contract
• Start date: October 19, 2026
