RN Case Manager
Position Overview
The facility's medical management department is seeking an RN Case Manager to provide direct and indirect services to patients and families. You'll identify and resolve psychosocial challenges, conduct comprehensive assessments, and collaborate with interdisciplinary teams to develop safe, timely discharge and treatment plans.
Key Responsibilities
- Complete initial and concurrent insurance reviews and skilled nursing facility placement assessments
- Conduct psychosocial assessments through patient/family interviews and chart review
- Develop, implement, and validate discharge and treatment plans in collaboration with the care team
- Ensure continuity of care and timely transitions
- Build and maintain community and provider relationships
- Participate in patient-focused conferences and departmental quality improvement initiatives
- Demonstrate knowledge of legal and ethical issues related to end-of-life care, informed consent, and advance directives
- Identify, report, and intervene appropriately in cases of abuse or family violence
- Provide psychosocial expertise to support team building and conflict resolution
Requirements
- Current New York State RN License
- Bachelor of Science in Nursing (BSN) preferred
- BLS Certification
- SCREEN Certification preferred
- Strong organizational and advocacy skills
Position Details
- Schedule: Monday–Friday, 7:00 AM–3:00 PM (8-hour shifts)
- Hours: 37.5 hours per week
- Contract Duration: 13 weeks
- Start Date: October 12, 2026
- Flu vaccine required (no exceptions)
- Cancellation Policy: 14 days written notice required
