Care Coordinator
Position Overview
The facility is seeking a Care Coordinator to manage care coordination and services for assigned patients throughout their episode of illness or treatment. You will collaborate with patients, families, physicians, and the care team to ensure optimal clinical and resource outcomes.
Key Responsibilities
- Assess, plan, intervene, and evaluate patient care using advanced clinical nursing skills
- Coordinate care in collaboration with the multidisciplinary team, including physicians and payors
- Evaluate and analyze quality and utilization cases, including admission/discharge criteria, observation stays, and 30-day readmissions
- Participate in ongoing outlier reviews
- Initiate appropriate clinical pathways based on admission diagnosis
- Act as an advocate and resource for patients and families throughout their entire episode of care
- Facilitate communication among providers and monitor services rendered
- May be assigned to support the Emergency Department as needed
Requirements
- Minimum one year of recent case management experience in an acute care setting
- Current California RN licensure
- Current CPR certification
Position Details
- Schedule: 7:00 AM – 3:00 PM
- Duration: 13 weeks
- Hours: 40 hours per week
- Start Date: September 21, 2026
