Care Manager – Acute Medical Setting
Position Overview
The facility is seeking an experienced Care Manager to join its acute medical team. You'll manage a caseload of up to 25 patients, performing care coordination and discharge planning in a fast-paced hospital environment serving adult and geriatric populations.
Shift & Schedule
- Days: Monday–Friday, 7:00 AM – 3:30 PM (5×8 hours)
- Start Date: ASAP
- Contract Duration: 26 weeks
- Weekend Requirement: Every other weekend (Saturday and Sunday), with a weekday off in exchange
- Holiday Commitment: Must work 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day
- No floating required
Requirements
- Experience: Minimum 2 years in acute medical care with Care Coordination and Discharge Planning
- License: California state license required at start (pending licenses accepted if licensed by hire date)
- Skills: Care coordination, discharge planning, computerized documentation, CMS guidelines knowledge, CPT coding, disease management, familiarity with HMOs and IPAs
- References: 2 references covering the last year within the past 3 years
- Travel Experience: Required

- Collaboration with interdisciplinary teams
- 40-hour orientation
- All RTO and scheduling requests must be submitted during the application process
- Potential to float between nearby facilities as needed
