Occupational Therapist – Home Health
Location: Alameda County, California
Schedule: 40 hours per week, Monday–Friday (day shift)
Contract Duration: 13 weeks

About the Role
The facility seeks an Occupational Therapist to provide home health services across Alameda County. You'll deliver skilled occupational therapy to patients in their homes, with a focus on functional independence and quality of life.
Key Responsibilities
- Prioritize and conduct start of care visits
- Supervise Occupational Therapy Assistants in the field
- Complete clinical evaluations, recertifications, and discharge planning
- Submit clinical notes and OASIS documentation within 48 hours of each visit
- Report start of care visits by the next business day
- Maintain clear communication with your clinical supervisor, intake, and office staff
- Be responsive during standard business hours
Requirements
- Active California OT License
- Minimum 1 year of home health experience
What You'll Receive
- 3 days of comprehensive training (1 in-house, 2 in the field)
- iPad and Ring Central phone system
- Mileage reimbursement at $0.655/mile (first to last patient)
- Paid parking and toll reimbursement
- Option to use a company car
Productivity
You'll maintain an average productivity of 30 points per week across a typical caseload of 8 visits per day. Overtime requires supervisor approval.
Territory may expand as needed.
