Physical Therapist – Home Health
Location: Sacramento, CA area
Schedule: 40 hours/week, Monday–Friday plus one weekend per month
Contract Duration: 13 weeks

About the Role
You'll deliver physical therapy services to patients in their homes across the Sacramento territory, with potential to expand your service area. This role combines direct patient care with supervisory responsibilities and requires strong time management and communication skills.
Key Responsibilities
- Prioritize and complete start-of-care visits, ensuring timely patient enrollment
- Supervise Physical Therapy Assistants and guide their clinical practice
- Submit next-day mandatory reporting of start-of-care visits
- Complete clinical notes and documentation within 48 hours of each visit
- Maintain regular communication with your supervisor, intake team, and office staff
- Remain responsive during business hours to address clinical needs
Requirements
- Valid California Physical Therapist License
- Minimum 1 year of home health experience

Productivity Expectations
Target: 30 points per week. Caseloads are structured to average 8 hours daily. Overtime requires supervisor approval.
