Occupational Therapist – Home Health
Position Overview
The facility is seeking an Occupational Therapist for home health services in a multi-county service area. New graduates are welcome; home health experience is preferred but not required.
Service Territory
You'll provide care across two designated areas:
- Area #6: Vacaville, Fairfield, Rio Vista, Suisun City, Dixon
- Area #7: Vallejo, Benicia, Napa, American Canyon, Yountville
Schedule & Compensation Structure
- Day shift, 7:00 AM – 7:00 PM
- 26-week contract
- Two compensation options available:
- Guaranteed Hours: 36 hours/week with required productivity of 27–30 points weekly. Willingness to travel outside the service area or work weekends to meet visit expectations.
- Non-Guaranteed Hours: Per-visit basis with flexibility to occasionally refuse patients. Weekend coverage may be required.
- All compensation covers drive time, visit time, documentation, and phone communication with patients and providers.
Clinical Scope
You'll perform OASIS assessments, routine visits, evaluations, re-evaluations, and discharge planning for homebound patients, including those with confirmed or suspected COVID-19 status.
Required Credentials & Clearances
- Current California OT License
- BLS Certification
- Valid driver's license, auto insurance, and vehicle registration
- Annual TB, Flu; MMR, Varicella, HepB, TDap (10-year), Drug screen, Physical
