Physical Therapist – Home Health
Position Overview
The facility is seeking a Physical Therapist for home health visits across a multi-county service area. You'll manage your own caseload, traveling to patient homes to deliver skilled physical therapy services.
Service Area
You'll cover Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point. Additional travel outside the assigned area may be required.
Schedule & Expectations
- Day shift (7:00 AM – 7:00 PM)
- 13-week contract
- Weekend coverage may be required
- Willingness to travel outside the service area to maintain caseload
- Productivity expectation: 27 points per week (36-hour guarantee)
- Visit types are assigned point values that convert to billable hours; points include OASIS evaluations, recertifications, routine visits, discharges, and follow-ups
Requirements
- Current California PT License
- BLS Certification
- Valid Driver's License with active auto insurance and registration
- Annual TB test, MMR, Varicella, HepB, Tdap (10-year), and Flu vaccination
- Drug screen and physical exam
- Willingness to see COVID-19 positive and suspected cases
