Physical Therapist – Home Health

Position Overview

We're seeking a Physical Therapist for home health visits across a multi-county service area in Northern California, including Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point. Travel outside the assigned area may be required.

Schedule & Duration

• Day shifts (7:00 AM – 7:00 PM)
• 13-week contract
• Weekend coverage may be required
• All requested time off must be disclosed at time of submission

Productivity & Compensation

This is a per-visit position with a guaranteed 36-hour week and 27-point productivity requirement. Visit types and point values will be clearly outlined at assignment start. Rate is all-inclusive and covers travel time, visit time, documentation, and patient/provider communication.

Required Credentials

• Current California PT License
• BLS Certification
• Valid Driver's License with auto insurance and registration
• Annual TB, MMR, Varicella, HepB, Flu tests
• Tdap (10-year validity)
• Drug screen and physical exam

Essential Requirements

• Willingness to see COVID-positive and COVID-PUI patients
• Flexibility to travel outside service area as needed to maintain caseload
• Reliable transportation
